This form does not guarantee that your request will be approved. The graduate team will make a decision on your request.

Please note that no location changes will be allowed during the current semester. If approved, the location change will be processed for the following semester. Please keep in mind that a location change will result in you receiving a new I-20 with an updated program date, and this will show a new program start date on your updated I-20.
